Implements #3557. `docker agent setup` gains a third path that registers
a custom OpenAI-compatible provider (endpoint, API format, API key
variable) in the user configuration. Registered providers are merged
into every loaded agent config so `docker agent new|run --model
name/model` resolve them, `docker agent models` lists their models from
the endpoint's /models route, and the agent builder documents them.

Also moves the models command's user-config pre-run to the parent
command so the bare `docker agent models` form honors the user config.
